Scholarship Guidelines and Application
1.) Eligibility:
- Enrollment in a Kentucky Postsecondary Institution during the
award period.
- Registration for a minimum of six (6) credit hours per quarter/semester.
- Undergraduate or Graduate status
- Minimum 2.75 cumulative GPA (on a 4.0 scale).
- Nomination by Disability Support Services Officer at the
applicant’s postsecondary institution.
2.) Criteria for Award: The Scholarship Selection Committee will make award decisions based on the following criteria:
- Well articulated goals
- Academic achievement
- Disability related leadership and service
3.) Requirements: The applicant is responsible for ensuring that all items are submitted to the Scholarship Selection Committee by March 28, 2008. (Mailed materials must be postmarked by the deadline date.)
- Completed application
- Personal statement
- Letters of support
- Transcripts of all postsecondary grades
* INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED*

5.) Notification: Scholarship applicants will be notified of their award status by May 2, 2008. Two $500 scholarships will be awarded and presented at the annual KY AHEAD Conference Dinner on May 20, 2008.
