AHEAD Professional Survey
Informed Consent
Thank you for taking time to provide valuable responses to the 2010 Disability in Higher Education Professional Survey. Completion of the survey will likely take between 10 and 15 minutes.
We recognize the sensitive nature of the questions included in this survey. Researchers and other staff at AHEAD will not be able to identify respondents by name, institutional affiliation, computer (IP) address, or e-mail address. Your answers to this survey will be completely anonymous.
After submitting your survey responses, you will be presented with a new screen that will ask for input that is *not* confidential or anonymous.
Responding to the questions on this final, post-submittal, screen will allow you opportunities to be entered in a random drawing for AHEAD-related prizes, and the opportunity to participate in a possible follow-up study. Responses to these last-screen questions are voluntary and will be recorded in a unique database, completely separate from survey data. Responses to the final questions will not be linked in any way to survey responses. Information provided in response to the final questions will be used only by AHEAD for the stated purposes, and will not be available to the AHEAD staff or Board or to the public. Additionally, your personal contact information, provided in the final questions, will not be shared with vendors or used for solicitations. These separate-screen questions are clearly marked so you will easily recognize them as being separate from the rest of the survey.
Survey results will be used in aggregate form. Raw data will not be available to any members of the public, members of AHEAD, or members of the AHEAD Board of Directors. It will only be available to the members of the research team engaged by AHEAD and will be stored in a secure computer database accessible only by password. Hard copies of data will be kept in a locked file for a period of ten years, at which time it will be destroyed.
**Voluntary Participation**
Your participation in this survey is entirely voluntary, and you may exit the online survey at any time and for any reason. You may also skip any question in the survey, for any reason. Your decision whether or not to participate will not affect relationships that you have with AHEAD in any way.
