She moves on to discuss how the new requirements flips the paradigm around by making the student narrative as the primary source of information while interaction, observation and professional documentation being secondary and tertiary respectively.

Adam Meyer describes how the 2012 AHEAD Documentation Guidance can be translated into usable, publishable office policy. He walks through the primary goal of collaborative conversation with the student, the secondary level of informed professional judgment, and the possible need for a third level in which documentation from a medical professional is necessitated not by relevance to policy, but by relevance to the particular student situation.
